FAA Issues SAIB for Diamond DA20 Nose Gear Cracks  
   The FAA recently issued a Special Airworthiness Information Bulletin (SAIB) regarding a potentially unsafe condition with the nose landing gear on the Diamond DA20 series aircraft.  The SAIB, issued December 22, warns of cracks discovered in the nose landing gear (NLG) fork assembly during routine inspections. In response, Diamond Aircraft Industries has revised their maintenance manual to require more frequent and detailed inspections. 
   The FAA recommends all affected owners and maintainers acquaint themselves with and adhere to the changes in Diamond's revised inspection schedule as well as replace their original NLG fork assembly with the improved part. NPRM Seeks Comments on Future Airspace Transition 
     On Dec. 15, 2011, the FAA issued a Notice of Proposed Rulemaking (NPRM) to seek comments on a proposed transition of the U.S. National Airspace System (NAS) navigation infrastructure to enable performance-based navigation (PBN) as part of the Next Generation Air Transportation System (NextGen). The FAA plans to transition from defining airways, routes and procedures using VORs and other legacy NAVAIDs towards a NAS based on Area Navigation (RNAV) everywhere and Required Navigation Performance (RNP) where beneficial. Since VORs do not enable RNAV, RNP, or Automatic Dependent Surveillance-Broadcast (ADS-B) operations, the FAA plans to reduce costs by drawing down the number of these FAA-provided VORs to a minimum operational network. 
     As part of this transition process, the FAA will form a working group to engage aviation industry stakeholders and other members of the public for input. The group will also provide a candidate list for VORs for discontinuance that will ensure safety and continuous operations. For details and instructions on how to provide comments, see:

Fwd: C-130 Gear Collapse





Keep your eye on the outboard engine on the right wing as the wingtip touches the runway and the prop strikes the pavement.  The prop mounting bolts shear off, the prop assembly separates from the driveshaft and the engine runs away in a cloud of smoke.  Just before that, something either ruptured a wing tank or cut a fuel line in the vicinity of the inboard engine, with a huge fireball that didn't turn into a major fire.
 
But the most impressive thing was how the pilots shut down the remaining three engines, simultaneously went to coarse pitch on the propellers, and got out of there, all in about 30 seconds.  I guess all the training kicks in, and you go on pure adrenalin.  Best in Full Screen mode.

Cessna Launches New Safety Initiative  
            Cessna Aircraft Company launched a new safety initiative this month to educate owners about new inspection procedures designed to help safeguard against corrosion and fatigue. The initiative affects 145,000 airplanes of the 100- and 200-series built between 1946 and 1986. The program stresses visual inspections for both corrosion and cracks caused by metal fatigue.
            The revised inspection program will be published in the airplane service manual in December 2011 for the 200-series, and in April 2012 for the 100-series. According to a Cessna engineer, the program is primarily a "visual process aimed at supporting the continued airworthiness of aging airframes." The criteria for inspections will vary by model and aircraft age or hours of operation.

This is fascinating; especially in light of the killing last week of al-Awlaki in Yemen .
Drone Controllers:
The female controller is actually flying the aircraft. Her flight instruments screen is located in the upper left
of the lower central large monitor screen. The other screens are views from the drone itself. Nicely done!
The control booths or rooms are made in Garland , Texas at Raytheon. For non-pilots, these controllers
are in Nevada and are each flying a drone thousands of miles away in the combat zone in Iraq and
Afghanistan .
Their left hand is on the throttle controlling the drone's engine.  Note all the buttons which perform
various tasks without removing the hand from the throttle.  The right hand is flying the plane.
Kill a Taliban leader then go home for dinner! Welcome to the new world.  This is modern warfare.

Bulletin Stresses Proper Performance at Idle for Cessna 172 
   On October 26, 2011, the FAA issued a Special Airworthiness Information Bulletin (SAIB) that addresses a concern with proper engine performance at idle for 1996 and after Cessna 172R and 172S models. Engine fuel servos on these aircraft have the potential to shift to an over-rich condition with the throttle at idle, which, if not addressed, could lead to the engine's inability to operate at idle-stop and possibly prevent a restart in-flight. The SAIB stresses the importance of understanding how an engine should properly perform when at idle, and how operators need to review, understand, and adhere to the operating limitations defined in the POH/AFM and maintenance manual. 
     Cessna has taken action to include information in their service documents and POH updates for these models to address the issue and improve the operator and maintenance communities' understanding of what proper engine performance should be with the throttle at idle. FAA Creates New Laser Webpage 
      In a continuing effort to combat the growing problem of lasers directed at aircraft, the FAA has created a new website to make it easier for pilots and the public to report laser incidents and obtain information on the subject. The website, which can be found at www.faa.gov/go/laserinfo, collects a wide array of laser information into one location. It includes links for reporting laser incidents, laser statistics, FAA press releases, and FAA research on the dangers lasers can pose to pilots, as well as downloadable videos.
Laser event reports have increased steadily since the FAA created a formal reporting system in 2005 to collect information from pilots. Reports rose from nearly 300 in 2005 to 1,527 in 2009 and 2,836 in 2010.
 
Venturing Further Afield 
   Airports—they come in all shapes and sizes. Some are small and sleepy, while others are abuzz with more activity than a hornet's nest. Despite the differences, airports can offer opportunities for education and enjoyment at all levels. In her article, "Venturing Further Afield," author and FAA Safety Briefing editor Susan Parson explores the many benefits of going "off the chart" when it comes to finding new places to park your airplane, not the least of which involves supporting our country's vital network of general aviation airports. Parson also touts the popularity of state-sponsored airport visitation programs as incentives for both students and "post-graduate" flyers looking to gain proficiency and/or earn higher certificates or ratings.

Fifth Annual International Helicopter Safety Symposium

Aviation authorities from more than a dozen countries across the globe will offer their perspectives on safety at the International Helicopter Safety Symposium taking place in Fort Worth, TX, on Nov. 8-9. The theme of the fifth annual symposium for the worldwide helicopter industry will be Human Factors: our Collective Challenge – Our Individual Responsibility.

The event is a gathering of helicopter operators, manufacturers, service providers, trade associations, academia, government regulators, pilots, mechanics, instructors, and other industry participants from across the globe aimed at finding ways to drive the helicopter accident rate lower. Our focus will be on human factors and how they affect helicopter safety. Hear from experts in human factors and pilot decision-making and learn about recommendations and strategies for mitigating operational risks.
 
Organizations involved in the symposium are also members of the International Helicopter Safety Team, created in 2005 with a mission of reducing the worldwide civil helicopter accident rate by 80 percent by 2016.

The two-day event will include discussions about helicopter safety and operations in numerous countries and international regions, including: Europe, the Persian Gulf Region, Australia, Brazil, Canada, India, Japan, Russia, and the United States.

CFI Runway Incursion Initiatives

As a CFI, you are aware of the FAA's goal to reduce runway incursions.  The following information conveys the seriousness with which the FAA is approaching this goal, and that you are a critical asset to the FAA to accomplish this goal.  A runway incursion poses the risk of tremendous loss of life and property, just as a mid-air collision would, and thereby demands the use of our highly professional designated examiners and CFIs.
 
Initiatives:

  • Our initiatives begin with including in all the Practical Test Standards a required Runway Incursion Avoidance TASK.  The first of the PTS to do so is the CFI PTS, followed by the Private and Commercial PTS.  The remainder of the PTSs will follow in due course.
  • To support the certification process, we initiated updating all the pilot certificate written examinations to include detailed Runway Safety material relative to airport signage, runway markings, ATC instructions, and pilot procedures to avoid a runway incursion.
  • To support this effort to train, test, and check pilots, detailed Runway Safety and Runway Incursion Avoidance informational material and procedures are being added to the Pilot's Handbook of Aeronautical Knowledge and the Airplane Flying Handbook.  These two initiatives provide you with the tools to train a pilot to ensure that they possess the knowledge and proficiency in the area of runway safety, and specific knowledge and skill to avoid a runway incursion.
  • We continue our initiative with rewriting FAA guidance in the conduct of remedial training for runway incursion cases.  The skills and expertise of a DPE will be employed for category A and B runway incursions, and a CFI associated with the FAASTeam will present runway incursion remedial training to category C and D runway incursions as appropriate.  To assist you in this activity, we have developed a standardized National Baseline Runway Safety/Runway Incursion Lesson Plan.  This lesson plan will be the basis upon which a remedial training program will be developed by a FAASTeam Program Manager in collaboration with the Regional Office of Runway Safety.  Again, we look to you to spread the important message to pilots that they must possess complete knowledge and understanding of the meaning of airport signage, runway marking, ATC instructions, and that they employ recommended pilot procedures to avoid a runway incursion.
  • It is recognized that a runway incursion, just as a mid-air collision, poses a tremendous risk of a horrific loss of life and property.  To provide procedures to enable pilots to avoid causing a runway incursion, two Advisory Circulars, AC-91-73, Single Pilot Procedures During Taxi Operations, and AC 120-74, Flight Crew Procedures During Taxi Operations, are being updated to provide pilots with the latest runway incursion avoidance procedures which every pilot must know and use.

ATLANTA HARTSFIELD-JACKSON INTERNATIONAL AIRPORT (ATL)

REDUCED DIVERGENCE AREA NAVIGATION (RNA V)

STANDARD INSTRUMENT DEPARTURES (S I Ds)

Beginning Thursday, October 20, 2011, ATL will implement new Reduced Divergence RNAV SIDs.

These SIDs have been designed to aid in noise abatement and add departure capacity to the ATL airport while ma i ntaining an equivalent level of safety when compared to existing separation standards. To achieve this, ATL will implement the Equivalent Lateral Spacing Operations (ELSO) standard developed by MITRE Corporation and approved for implementation by FAA Waiver I1-T-OS.

 

1. Reduced Divergence RNAV SIDs will be conducted daily between 0700-2300 Local Time .

Between 2300-0700 Local Time, aircraft will be issued noise abatement radar vectors.

 

2. Pilots must ensure that they are operating on the current RNAV da t abase (dated 10/20/20 II or   later) and that the correct departure runway is entered into the Flight Management Computer (FMC) prior to departure.

 

  NOTE- Several  fix/waypoints on the new SIDs are new or have moved from their previous location .

 

3. ATL controllers will issue departure clearances to RNA V aircraft using the "RNAV to"

phraseology. Example - "DALl23, RNAV to SNUFY, Runway 26L , Cleared for Takeoff." I n

this case, SNUFY would be the first named fix/waypoint on the SID to be flown. The pilot is

expected t o verify that the fix/waypoint displayed on the Flight Management System (FMS)

corresponds to the fix/waypoint issued in the departure clearance. If the fix/waypoint is not

identical, pilots must request a vector from the Tower.  

 

4. Advise ATC immediately of any RNAV anomaly and request vectors as necessary.

 

5.  Extra vigilence is required in order to ensure the highest level of safety. The ATL Reduced Divergence RNAV SIDs commonly use divergence less than 5-degrees as depicted in Figure #1 and #2 (attached.) Departure tracks may be separated by as little as 10-degrees.
